New Patients & Forms

To save you time at your first appointment, we encourage you to download our new patient forms prior to your visit. Once your forms are downloaded, you can type the information in and email it directly to us or fill it in by hand and fax it to (602) 923-8502. If you choose to fax or email the froms, we will have you sign them at our office prior to your appointment.

Please make sure you also have the following items with you:

1. Drivers License or government picture ID
2. Proof of Insurance
3. Referring physician order (referral if applicable)
4. Any x-rays or test results related to your medical condition (if possible)

PAYMENT INFORMATION

We accept most major insurance plans, credit cards and workman’s compensation. Deductible, co-pays, co-insurance, and/or non-covered services are due at time of service. We accept cash, checks, American Express, Discover, Visa and Mastercard.
